Independent Pharmacy Operations Platform
Pharmacy AI automates prior authorizations, refill outreach, adherence monitoring, and claim management -- so your team spends time on patient care instead of paperwork.
The Problem
CVS and Walgreens each operate 40-person back-office teams handling prior authorizations, patient outreach, and claim appeals. Independent pharmacies have you, one technician, and a fax machine that overheats on Mondays. The result is predictable: patients abandon prescriptions, PAs sit unanswered for three days, and adherence gaps quietly convert loyal customers into chain regulars. Pharmacy AI closes that operational gap without a single new hire.
Platform Capabilities
Pharmacy AI integrates via HL7 FHIR or a direct API bridge with PioneerRx, QS/1, Liberty, and Rx30. No rip-and-replace, no downtime, no retraining staff on a new system.
Pharmacy AI reads the PA requirement from the PBM response, drafts the clinical justification from the patient's medication history and diagnosis codes, and submits via the payer's preferred channel -- fax, portal, or API. Status updates land in your dashboard instead of your inbox. When a PA is denied, the system flags the most likely appeal pathway and drafts the supporting letter for pharmacist approval in one click. No more PA purgatory.
Three days before a patient's calculated supply runs out, Pharmacy AI sends a personalized SMS or voice outreach -- patient's preferred channel, set at enrollment. Message cadence is tunable per medication category: high-adherence chronic medications get a tighter outreach window than PRN medications, which are automatically excluded. Patients confirm, decline, or request a callback. Your team sees a clean confirmed-refills queue each morning instead of a stack of manual callbacks.
Pharmacy AI scores each active patient's medication adherence rate across their therapy classes, updated daily. Patients who fall below 80% PDC on a critical medication trigger an alert before the 90-day measurement window closes -- giving your pharmacist time to intervene before the patient falls out of the DIR fee calculation. Scores are exportable for MTM billing documentation and retroactive DIR adjustment appeals.
Every rejected claim is captured, categorized by rejection code, and scored by likelihood of successful appeal. Recoverable claims get a prioritized queue with a pre-drafted reversal message. Time-sensitive appeals -- those with 12-day BIN-level windows -- are flagged immediately with a countdown. Expired appeal windows are logged automatically for DIR challenge purposes. Early customers recover an average of $2,100 in previously abandoned claim revenue within their first 60 days.
Compounding pharmacies face intensifying USP 795 and 797 documentation requirements. Pharmacy AI generates batch records, beyond-use dating labels, stability data references, and facility cleaning logs from a structured template library. Your pharmacist reviews and signs each record; the system assembles it. Audit-ready PDF packets are stored and indexed so that NABP and state board inspections do not require 48 hours of manual retrieval from stacked binders.
All patient-facing outreach -- refill reminders, copay assistance alerts, new prescription status, and consultation scheduling -- flows through a single HIPAA-compliant inbox. Messages touching clinical content are held for pharmacist review before sending. Every communication is logged against the patient record. All messages route through our BAA-covered messaging infrastructure: never your personal cell, never a generic short code that looks like spam.
Getting Started
Your existing PMS stays in place. Pharmacy AI reads from it and augments it -- nothing is replaced, nothing migrated, no workstation installs required.
Share read-only credentials for your pharmacy management system. Our onboarding team configures the HL7 or API integration in under four hours. PioneerRx, QS/1, Liberty Software, and Rx30 are all supported with same-day setup. Your data never moves; Pharmacy AI reads it in place.
Choose which modules to activate and set your pharmacist approval thresholds -- which message types require your sign-off, which go out automatically, which medication classes get a tighter refill outreach window. Most pharmacies go live with defaults and fine-tune during the first two weeks based on real patient response data.
From day one, your PA queue shrinks, your refill confirmation rate climbs, and your morning routine shifts from reactive firefighting to reviewing a clean to-do list the system already prepared. Ninety-day outcomes consistently include measurable improvements in PDC scores, claim recovery rates, and net patient retention.
Who This Is For
Pharmacy AI is designed for independently owned pharmacies with 1-5 locations, 150-600 scripts per day, and a lean team that handles operations alongside dispensing. If you have a dedicated IT staff and a workflow automation team, you do not need us. If you are the pharmacist, the owner, and the PA coordinator all at once -- you do.
Independent pharmacy owners who handle PA calls personally and know which three PBMs account for 80% of their daily friction.
Multi-location independents (2-5 stores) that want consistent operational workflows across locations without hiring a dedicated operations coordinator for each site.
Compounding pharmacies under increasing USP 795/797 compliance pressure who need documentation automation without paying for a full QA software license.
Long-term care pharmacies managing high-volume, high-complexity medication synchronization for facility residents where adherence gaps have direct clinical consequences.
Early Results
"We were spending three hours a day on PA calls. Within two weeks of going live, that dropped to maybe 30 minutes of review. The system drafts the letters, I just approve them. It is the single biggest time reclaim I have found in 18 years of owning this pharmacy."
"Our adherence scores were tanking our DIR fees. Pharmacy AI flagged 41 patients in the first month who were about to drop below the PDC threshold. We intervened on 38 of them. I can show payers a documented adherence program now, which matters in our next contract negotiation."
"Our compounding binder used to be 400 pages of handwritten batch records. Pharmacy AI generates everything from our formula templates. The state board inspection last month took 90 minutes instead of a full day of me digging through binders. Worth every penny."

FAQ
Which pharmacy management systems does Pharmacy AI integrate with?
Current integrations are live with PioneerRx, QS/1 (Omnicell), Liberty Software, Rx30, and BestRx. Integrations with RxSafe and McKesson Pharmacy OS are in closed beta. If your PMS is not listed, contact us before assuming incompatibility -- most modern systems support HL7 FHIR export or a structured flat-file format that our team can bridge.
Is Pharmacy AI HIPAA compliant?
Yes. We sign a Business Associate Agreement with every customer before any data sharing begins. All PHI is encrypted in transit and at rest using AES-256, stored in US-only infrastructure, and never used to train or fine-tune any model. Our privacy officer is available directly to answer questions from your compliance team or state licensing board.
Does the AI send clinical messages to patients without pharmacist review?
No. Refill reminders and routine administrative messages go out automatically once you configure the schedule. Any message that touches clinical content -- a medication change, a PA status requiring patient counseling, an adherence gap that warrants a pharmacist call -- is held in a review queue and does not send until a pharmacist approves it. You set the content thresholds that trigger review.
What does setup involve? Do I need to hire an IT contractor?
No external IT is needed. Our onboarding team handles the PMS connection using the read-only credentials you provide. The process takes two to four hours and requires no downtime and no software installation on your workstations. Everything runs in the cloud. Most pharmacies are fully operational within 24 hours of providing credentials.
Can I try it before committing to a paid plan?
Yes. All plans start with a 30-day free trial on one location. No credit card is required for the trial period. Any claims recovered and adherence improvements generated during the trial are yours to keep regardless of whether you continue. We would rather you see real outcomes before you pay than sign up based on a sales presentation.
Does this replace my pharmacy staff?
No. Pharmacy AI handles the paperwork, the outreach, and the queue management. Your pharmacist and technicians handle the counseling, the clinical decisions, and the patient relationships that cannot be automated. The goal is that your team stops losing half their day to tasks that should not require a licensed pharmacist. The clinical judgment stays with you.
